Similar to how Catholics adopt a second middle name of a saint or biblical person. I don’t believe Ruthie was a convert but in fact an Ashkenazi Jew. Ashkenazic custom is to not name a baby after living individuals. Also if a new wife has the same name as the husband’s mother a living person, the wife’s name should not be changed but should not be used as the main one. This may explain the middle/first name swap.
